Nurse Practitioner – Saltville Community Health Center (Saltville, VA)
Saltville Community Health Center is seeking a dedicated Family Nurse Practitioner to join our primary care provider team. This role delivers comprehensive patient care—assessment, diagnosis, treatment, follow-up, and patient education—under the general supervision of a licensed physician. The ideal candidate is collaborative, patient-centered, and committed to high-quality care in a community health setting.
Key Responsibilities
Patient Care
- Conduct complete histories, physical exams, assessments, and diagnoses.
- Develop and manage treatment plans; prescribe medications per protocols.
- Order and interpret labs, imaging, and diagnostic tests.
- Manage acute and chronic conditions and provide emergency care as needed.
- Educate and counsel patients and families on health conditions and treatments.
Clinical & Team Collaboration
- Consult with supervising physicians and interdisciplinary team members.

Dental Hygienist – Tazewell Community Health Center (Tazewell, VA)
Tazewell Community Health Center, part of Southwest Virginia Community Health Systems (SVCHS), is seeking a dedicated Dental Hygienist to join our patient-centered dental team. This role supports high-quality clinical care, preventive education, and outstanding patient experience across our community-focused practice.
Key Responsibilities
- Perform scaling, root planing, and clinical examinations under dentist direction; assist with diagnosis by charting caries, periodontal pockets, and abnormal conditions.
- Administer intraoral block/infiltration anesthesia or nitrous oxide (18+ years old) with required Board-approved training, exam, and current BLS/ACLS certification.
- Independently complete routine scaling, polishing, charting, and preventive hygiene services under general supervision.
- Deliver preventive education, home-care instruction, and nutrition counseling to support oral health.
- Take accurate dental radiographs; place cavity-preventive agents as prescribed.
- Document all procedures and maintain thorough, compliant patient records.
- Follow OSHA, Bloodborne Pathogen, and universal precautions; ensure treatment rooms and instruments meet sterilization/cleanliness standards.
- Take blood pressure/pulse readings; update medical and dental histories at each visit.
- Assist with equipment maintenance, hygiene supply management, and sterilization workflows.
- Participate in scheduling, patient communication, recall processes, and occasional front-office support.
- Support dental hygiene students during clinical training and collaborate in team meetings.
- Perform duties under remote supervision when applicable, including exams, x-rays, scaling/polishing, fluoride application, and required documentation.
Qualifications
- Licensed Dental Hygienist in the Commonwealth of Virginia.
- Certification to administer local anesthesia and/or nitrous oxide (or ability to obtain).
- Current BLS or ACLS certification.
- Strong commitment to patient care, safety, compliance, and teamwork.
- Ability to work effectively under direct, general, or remote supervision per Virginia Board of Dentistry regulations.
Migrant Health Outreach Worker (Carroll, Patrick, Wythe Counties)
We are seeking a dedicated Migrant Health Outreach Worker to serve farm workers across Carroll, Patrick, and Wythe counties. This role works in the field, independently locating and serving farm workers, while building strong relationships with growers, agricultural communities, and clinic teams.
Position Summary
The Migrant Health Outreach Worker acts as a liaison between the community health centers and migrant/seasonal farm workers. This role provides outreach, health assessments, case management, transportation, interpretation/translation, and health education. The Migrant Health Outreach Worker documents all activities and maintains collaborative relationships with growers, community partners, and farm worker families.
Outreach Activities
- Locate and visit farm worker camps and agricultural worksites within assigned counties.
- Conduct health assessments and identify healthcare needs.
- Inform farm workers about available clinic services and community resources.
- Maintain a Continuity File including grower lists, camp details, and community partner contacts for future staff continuity.
Case Management
- Interview farm workers to determine needed services.
- Assist individuals in accessing medical care and community services through referrals, advocacy, and follow-up.
- Provide interpretation and translation assistance as needed.
- Provide or arrange transportation when no other resources are available.
Health Education
- Deliver health education at worksites, camps, or clinic locations.
- Teach or coordinate prenatal health education sessions when needed.
Other Expectations
- Maintain strong working relationships with growers, farm workers, and SVCHS staff.
- Document all activities accurately and promptly.
- Perform other related duties as assigned to support program and organizational goals.
- Must be able to adapt to schedule changes or location assignments as directed by leadership.

Remote Behavioral Health Therapist
We are seeking a skilled and compassionate Behavioral Health Provider to join our multidisciplinary team. This role delivers behavioral health services both onsite and via telehealth and provides consultation support to primary care providers.
Position Summary
The Behavioral Health Provider is responsible for assessing and treating patients and families with mental health and behavioral health needs. This role delivers individual, group, and family treatment, supports integrated care planning, and assists primary care providers with behavioral health consultation. Services are provided within SVCHS facilities and through telehealth platforms.
Key Responsibilities
- Conducts behavioral health screenings and assessments.
- Provides outpatient therapy using individual, group, and family modalities (in-person and telehealth).
- Works collaboratively with primary care providers to recognize, diagnose, and treat behavioral health concerns.
- Supports patients in improving coping skills and reducing symptoms related to chronic conditions (e.g., diabetes, chronic pain, hypertension).
- Participates in integrated care team activities including treatment planning, team meetings, and case discussions.
- Maintains accurate, timely documentation in accordance with SVCHS medical records policies.
- Adheres to confidentiality laws, policies, and ethical standards.
- Meets organizational productivity standards established by the SVCHS Board.

Qualifications

Southwest Virginia Community Health Systems, Inc. is looking for a Registered Nurse/Licensed Practical Nurse Care Coordinator specializing in primary care to work in a medical facility in Bristol, VA and Tazewell, VA
Work Hours

Job Description
- The candidate will work on a multidisciplinary healthcare team in a primary care setting and focus on coaching and coordination of care for high-risk, chronically ill patients and those with co-morbid conditions.
- The candidate will work alongside the providers and office staff in a Patient-Centered Medical Home (PCMH) environment, potentially serving multiple locations.
- The candidate will provide care coordination services for patients and assist the practice to implement systems changes to improve quality and access to care, and reduce hospital admissions, ER visits, and overall healthcare costs.
- The candidate will actively participate in multidisciplinary patient-centered team meetings and develop and strengthen community partnerships.
- The candidate will assist providers and other care team members in implementing processes for best practices for preventive services, chronic care and disease management.
- The candidate will utilize EMR /registries reporting to identify high-risk patients, proactively engage patients, ensure they maintain a connection to the medical home, conduct pre-visit planning with providers such as printing daily huddles sheets and risk assessments and prioritize patient follow-up.
- Work collaboratively with providers and the care team to ensure development of the plan of care in partnership with the patient and family, including all appropriate preventive and disease-specific screenings, interventions, treatment goals including self-management goals
Qualifications/Experience:
- Certification of practice as a Registered Nurse, Licensed Practical Nurse
- Certified Case Management (CCM) certification*. Preferred as available
- A minimum of five (5) years’ experience in health center setting
- Proficient computer skills

Pediatrician – Tazewell
The Pediatric staff physician works as part of the medical provider team providing medical services to the pediatric patients of SVCHS. In addition, staff physicians, unless specifically exempted by the CMO, shall be qualified to serve on the active medical staff of a local hospital, assume “on call” after hours responsibilities and supervise community health center PAs or NPs as appropriate.
Qualifications:
– Graduation from an accredited medical school with a MD or DO degree.
– Completed residency and Board Certification preferred.
– Licensed in VA.
– The ability to relate with warmth & effectiveness to patients and other health care providers of
SVCHS.
– Must be ACLS qualified.
– Ability and willingness to serve on the active medical staff of a hospital used by SVCHS.
