Medication Assistance Program (MAP) to Assist Qualifying Community Members in Obtaining Eyeglasses

Southwest Virginia Community Health System’ Medication Assistance Program (MAP) can now help qualifying children and adults in receiving free eyeglasses.

Eyeglasses are provided free of charge with no additional costs to the qualifying patient. Adults qualify for one pair of eyeglasses per year and children qualify for two pair per year.

The Medication Assistance Program (MAP) is designed to help supply free medications for Southwest Virginia residents who are either without prescription coverage or who are currently in the Medicare Coverage Gap “donut hole.” These medications are provided by using The Pharmacy Connection, which has direct contact with pharmaceutical companies.

If eligible, the Medication Assistant Coordinator will assist with all paperwork required for applying for medications.

Eligibility for MAP is determined by the companies who provide the medication requested. There are no guarantees that patients will qualify for assistance. Not all medications prescribed are offered on this program. If medications are available and patients qualify for assistance, it may take 4-8 weeks or longer to initially receive medications.

Participants do not have to be a patient of SVCHS, but a Primary Care Physician would have to be willing to sign their applications.

Please contact Connie Eastridge at 276-496-4492 ext. 1112 for more information on this program.

SVCHS does have two optometrists on staff at Saltville Community Health Center in Saltville, Va. and Tazewell Community Health Center in Tazewell, Va. that can assist with a comprehensive eye exam or soft contact lens fitting.

In addition to addressing SVCHS’ optometrists addressing visual complaints, they have strong emphasis on medical conditions such as cataract co-management, conjunctivitis, diabetic eye disease, dry eye syndrome, glaucoma, and macular degeneration.
