Southwest Virginia Community Health Systems Receives National Award to Advance Innovative Care for Chronic Kidney Disease

Southwest Virginia Community Health Systems (SVCHS) has been named one of only four recipients nationwide of the 2025 BD Helping Build Healthy Communities Innovations in Care Award. This prestigious recognition includes a $225,000 grant to support SVCHS’s efforts to expand pharmacist-led chronic kidney disease (CKD) management for patients across the region.

With this funding, SVCHS will enhance its existing clinical pharmacy workflows by increasing screening efforts, optimizing evidence-based care, and addressing social determinants of health (SDOH) that impact patient outcomes. The initiative centers around comprehensive medication management, driven by clinical pharmacists working as part of a collaborative care team.

“Thanks to grant funding from Direct Relief and BD’s Helping Build Healthy Communities initiative, we are expanding our clinical pharmacy services to better support patients with chronic kidney disease. This effort reflects our commitment to whole-person care and to breaking down barriers so that all patients can access the support they need.”

About the Award Program

Since 2013, BD (Becton, Dickinson and Company) has partnered with Direct Relief and the National Association of Community Health Centers (NACHC) to support community health centers across the U.S. that care for low-income and uninsured patients.

To date, the BD Helping Build Healthy Communities initiative has supported 56 health centers across 20 states, with more than $22.6 million in total investments through cash and product donations.

The Innovations in Care Award recognizes health centers that are advancing innovative models of care while addressing social and medical needs—such as transportation, food access, housing, chronic disease management, and preventive care.

Looking Ahead: Improving Chronic Disease Outcomes

SVCHS will use the grant to expand its pharmacist-led chronic disease management services, particularly for those at risk of kidney disease. Patients will receive education, counseling, and medication support tailored to their specific needs and health goals.

“This initiative strengthens our ability to deliver targeted, preventative care that truly makes a difference in people’s lives,” added Land. “We’re grateful to BD and Direct Relief for investing in community-based healthcare solutions that meet patients where they are.”
